相づちの表現で、相手の意見などに対して深い共感や理解を示すときに使います。

I have to take time off work and everything to be there.

「こっちは仕事も何もかも放ったらかしにして、長時間そこにいなきゃいけないのよ」
"take time off"で「時間を取る」今していることを中断して、かなり長い時間、休憩や他のことに時間をかけるということです。短い時間の場合は"take a break"と言います。
ex: Teddy took time off his hectic schedule to guide us around the vessel.
（テディは超多忙なスケジュールを割いて、私たちを連れて、その大型船を案内してくれた）

It's so frustrating sitting in the waiting room twiddling your thumbs

「何もすることなく待合室で座っているのって、すごくイライラすることでしょ」
"twiddle one's thumbs"は直訳すると「親指を回す」。両手の指を組んで親指を回すということで「ムダに時間を過ごす」という意味のイディオムです。何もすることがなくただ待っていて、手持ち無沙汰でイライラしている状態を表します。
ex: The airplane was delayed because of bad weather, and I had to sit around for 2 hours in the airplane twiddling my thumbs.
（悪天候のために飛行機が遅れて、飛行機の中で2時間も、ぼんやりとムダな時間を過ごさなくちゃいけなかったんだ）

the doctors are run off their feet, too.

「お医者さんはすごく忙しい」
"run someone off his feet"で「（人）を忙しく働かせる、駆けずり回らせる」という意味のイディオム。類似表現として、"run"の代わりに
"rush"を用いた"rush someone off his feet"、また"feet"の代わりに"legs"を用いた"run someone off his legs"などもあります。
ex: During the event period, staff were run off their feet with around 300 inquiries each day.
（イベント期間中、スタッフは毎日300件ほどの問い合わせで大忙しだった）

I come back with steam coming out of my ears.

「かんかんに腹を立てて帰る」
"with steam coming out of one's ears"は直訳すると「耳から湯気を出して」。怒りで興奮している状態を示す表現です。
"〜 coming out of one's ears"は、「（人が）あり余るほどの〜を持っている」という意味でも使われます。
ex: Mark hit the lottery jackpot and got money coming out of his ears.
（マークは宝くじで大当たりして、あり余るほどの金を手に入れた。）
