Nurse: Just have a seat here, and we’ll get you registered. Let’s start with your full name and address.
Stan: My name is Stan Lampard. I live at 36 Ulster Rd.
Nurse: All right, Mr. Lampard. May I have your health card?
Stan: Sure, here you go.
Nurse: Okay. Do you have any allergies to medication of any kind?
Stan: Not that I’m aware of.
Nurse: Okay. And what seems to be the problem?
Stan: I was at work and I accidentally hit my hand with a hammer. I think I broke a couple of fingers.
Nurse: All right. Have you experienced any other symptoms?
Stan: Besides the blinding pain in my hand?…No.
Nurse: All right. Just have a seat over there and the doctor will be with you soon.
Stan: Thanks.

「どうぞ」
"Here you go"は、ものを差し出すときに良く使う表現です。"Here you are"という表現ももちろんつかえます。また、"Here it is"は、相手が探していたものを見つけて差し出すときに使う表現です。
ex: A: Do you have any baggies for these.
B: Sure. Here you go.
A: これらを入れるための袋ってありますか？
B: ええ。どうぞ。
Do you have any allergies to medication of any kind?
「特定の薬に対してなにかアレルギーはお持ちですか？」
海外旅行などでお医者さんにお世話になってしまうときに備えて、ちゃんと覚えておきたい表現です。その他の言い方として "Are you allergic to any medication?"があります。

「自分が知ってる限りはないですね」
これは"I do not have any allergies that I'm aware of"の省略形。
Besides the blinding pain in my hand?
「手の激痛以外にですか？」
"besides"は「〜に加えて」という意味の副詞。この場合の「〜に」は、直前の"symptoms"をさします。また、"blinding pain"は「激痛」ということ。
ex: I can't talk to you. My girlfriend is here and besides I don't want to talk to you.
