I: Hi Lance! You’re finally out of hospital! How do you feel?

L: Like a million dollars! It’s great to be free of all that pain.

I: I can imagine. I had a lot of trouble with my back a few years
ago, and it was hell.

L: Tell me about it. I’m so glad I went ahead with the operation.
I feel like a new man.

I: That’s great to hear. But I guess you have to take it easy for
a while, right?

L: Yeah. I’m dying to go and play a couple of rounds of golf, but
my doctor was pretty strict about not overexerting myself over
the next few weeks.

I: Well, when you’re 100% again, give me a call. Bob and I won a
weekend pass to the Meadowlands Golf Course and Resort, and we
can have up to four people in our group.

L: Meadowlands? Wow, I hear that course is international standard.

I: Yeah. I can’t wait to go, so hurry up and get better soon, OK?

L: You can count on it!

"like a million dollars"は「素晴らしい、絶好調で、とても魅力的で」
という意味。"look a million dollars"になると「(女性が)とても
魅力的に見える、とても元気に見える」という意味になる。
Ex: I feel like a million dollars today!

"be free of 〜"は「(負担、制約、不都合なこと)のない、
(税金、料金などを)免除されている・課せられない」という意味。
他に、“free"を使ったイディオムには、"free and easy"「屈託の
  ない、くつろいだ」、“feel free to 〜"「自由に〜してよい」、
  "get free of 〜"「〜を離れる、〜免れる、〜を脱する」、"make
free with 〜"「〜を勝手に使う、〜になれなれしくする」などがある。
Ex: It's nice to be free of worries at last.

"go ahead with 〜"は「(計画、仕事など)を進める、強行する、
推進する」という意味。“go ahead"は「先へ進む、取り掛かる、
さあどうぞ、(電話で)どうぞお話下さい、(人に先を譲るとき)
どうぞお先へ、トントン拍子に進む」などの意味がある。
Ex: I'll be free in January so you can go ahead with planning
the trip.
(私は、1月が空いているので旅行計画を進めていいよ。)

I feel like a new man.

(生まれ変わったみたいだ。)
"feel like a new man"は直訳すれば、「新しい人のように感じる」
という意味なので、「別人のように感じる、生まれ変わったみたい、
すっかり良くなる」という意味になる。他に、"man"を使った
イディオムには、"as one man"「いっせいに、揃って」、"to the
last man"「一人残らず、満場一致で」、"a man of people"
「大衆の見方」などがある。"man"は、男性しか使えない。
  女性なら"I feel like a new person"になるので注意が必要!
Ex: I was in hospital for three months, but now I'm out, I feel
like a new man.
   (私は3ヶ月入院したけど、退院してから生まれ変わったみたいに
    元気です。)
