Melissa and Tony are talking about Tony's girlfriend Samantha.
Melissa: How are things with Samantha going, Tony?
Tony: Yeah, they’re fine, I guess.
Melissa: That doesn’t sound very enthusiastic. What’s wrong?
Tony: Well, its just that she’s starting to get on my nerves a bit.
Melissa: But she’s such a sweet girl! What’s she doing that’s so bad?
Tony: She has this habit of constantly calling me when I haven’t returned one of her messages. There are times when I’m just too busy to get back to her straight away, but she keeps calling and calling. I’m getting sick and tired of it!
Melissa: Have you told her how you feel?
Tony: I’ve explained to her time and time again that sometimes I can’t call her back until later, but it doesn’t seem to be sinking in.
Melissa: Hmmm, that is a bit of a problem isn’t it? It sounds like you
need to sit her down and have a bit of a talk.
Tony: Yeah, that’s what I was afraid of.

What's wrong?
「問題でもあるの？」
相手の様子がおかしかったり、具合が悪そうなときに"What's wrong with you?"と声をかけます。ここでは"(Is there)Something wrong?"と言い換えることも可能。
its just that she's starting to get on my nerves a bit.
「ちょっと迷惑になりつつある」
"nerve"は「神経」とい意味で、"get on one's nerves"で、「神経に触る、イライラさせる」となります。インフォーマルな表現。
ex: There were times when I got on her nerves and she got on mine.
（僕が彼女をイラつかせることもあったし、その逆もあった）
I'm getting sick and tired of it!
「これには参ってるんだ！」
"be sick of 〜"も"be tired of 〜"もほぼ同じ意味で「〜にうんざりして」。この2つが"be sick and tired of 〜"と合体し、意味がさらに強調される表現です。
ex: I'm sick and tired of those annoying pop-up ads.
（ポップアップ広告にはうんざりだ）
it doesn't seem to be sinking in.
「分かってくれてないみたい」
"sink"は「沈む、染み込む」という意味の動詞で、そのイメージから"sink in"という熟語で「ちゃんと理解される」となります。
ex: The news from the President's staff took a little while to sink in

Tony: うん、ぼくもそう思ってたんだ。
