Amelia goes to visit her doctor about a problem she is having.
Amelia: I appreciate you seeing me like this on short notice, doctor.
Doctor: Don’t mention it. So, how can I be of help?
Amelia: Well, doctor, I’ve been having trouble sleeping lately.
Doctor: I see. Any idea about what might be causing it?
Amelia: Yes. I have an idea about what’s causing it, but I feel embarrassed about bringing it up.
Doctor: There’s nothing to be ashamed of. Insomnia’s quite common, and it can be brought on by a whole host of factors.
Amelia: I don’t think my condition really counts as insomnia.
Doctor: But, you’re having trouble sleeping?
Amelia: Yes. But the problem isn’t me…look, doctor, I love my husband, but he snores louder than anybody I know. I was used to it before, but it’s gotten worse lately and it’s keeping me up at night.
Doctor: You know usually if you nudge the person they –
Amelia: I’ve tried that. Believe me. It doesn’t work. Every time I try it, my husband makes this awful choking sound and starts snoring even louder…Doc, I’m exhausted…I’m at the end of my rope. How can I get him to stop?
Doctor: I’m not very experienced with this kind of case. But it sounds like the only solution to the problem would be to sleep in separate bedrooms.
Amelia: We have a one bedroom apartment. Isn’t there a surgery you can do or something?
Doctor: Well, I could refer you to a specialist, and perhaps there is some procedure that your husband can undergo. Although, I imagine such an operation would be complicated, and would involve several days stay in the hospital.
Amelia: Really. Any chance they’d admit him by tonight?

「急な連絡で時間をとっていただき、ありがとうございます」
"on short notice"は「急な（お願い）、間際に」という意味。似たような表現で"at short notice"がありますが、こちらは「何かをすぐにしなくてはならない」、というニュアンスが含まれます。
ex: Reservations are recommended, but sometimes a room is available on short notice.
（ご予約をお薦めしますが、急の場合でも部屋をおとりすることが可能なこともあります）
I feel embarrassed about bringing it up.
「言うのが恥ずかしくて」
"bring up"は、「持ち上げる」という意味で覚えている方も多いと思いますが、そこから転じて「話題を持ち出す、〜について話す」という意味にもなります。もちろんここでは後者の意味で使われています。

「不眠症は珍しくありませんし、多くの要素が原因となります」
映画の題名にもある"Insomnia"。ストレスが多い現代社会では、覚えておくべき単語です（覚えるのに苦労して不眠症にならないように）。
"bring on"は、「(病気や発作を)引き起こす」。
また、"whole host of"は「たくさんの」。"a lot of"と同じです。
ex: I don't know what brought on his sudden anger.
（彼がどうして急に怒り出したか分からない）
I'm at the end of my rope.
「これ以上どうすることもできません」
"at the end of one's rope"は、「綱の先端で（それ以上進めない）」から転じて、「窮地に陥って、万事休して」という意味。
ex: I am at the end of my rope about what to do about my current situation at work.
（会社での現状に対して何をするべきか、行き詰ってるんだ）

Amelia: それも試してみました。でも、なにも変わらないんです。軽く突くたびに、主人はむせぶようなすごい音を出して、さらに大きないびきをかき始めるんです。先生、もう疲れました。これ以上どうすることもできません。どうしたら彼のいびきはおさまるのでしょうか?
Doctor: こういったケースはあまり経験がないんですよ。でも、唯一の解決策として考えられるのは、別々の部屋で寝ることくらいでしょうか。
Amelia: 寝室が一部屋のアパートなんです。手術かなにかできませんか?
Doctor: 専門家に話してみますよ。もしかしたら、ご主人さんが受けることのできる処置があるかもしれません。でも、そういった手術は複雑で、数日間の入院が必要になるかもしれませんよ。
Amelia: ホントですか。今晩までに主人を入院させてくれるでしょうか?
